TRAM Flap Reconstruction
Candidates
- Individuals who have a loss of breast tissue and skin following mastectomy
on one or both sides may decide to have breast reconstruction using
a TRAM flap.
Intended Result
- The objective of this type of surgery is to reconstruct or to replace
breast tissue.
Procedure
- A flap of lower abdominal wall fat, muscle, and skin is transferred to
the area of missing breast tissue with its own blood supply. In a standard
TRAM flap, the blood supply is within the rectus muscles, which is left
attached at the lower edge of the rib cage. In a free TRAM, the blood
vessels are reconnected to the vessels in the axilla.
- The abdominal donor site is closed similarly to a tummy tuck, leaving a horizontal scare and a tighter abdomen.
- An attempt is made to create a “normal” breast mound and shape during the first operation, but subsequent smaller procedures are frequently necessary.
- Nipple/areolar reconstruction is performed at a later date.
Healing Process
- A TRAM flap breast reconstruction is a major surgery performed in the
hospital, requiring several days of hospitalization.
- Ambulation will begin the day following the surgery and will be reasonably comfortable within 10 to 14 days.
- Complete recovery from surgery usually takes approximately 6 weeks.
- The abdominal donor site tends to cause patients the most discomfort during the healing process.
Additional Enhancement
- An additional procedure that will enhance the result is a breast enlargement, breast lift, or reduction of the opposite breast, and nipple/areolar reconstruction.
Insurance Guidelines
- Breast reconstruction is almost always covered by insurance.
